Financial Aid Textbook Ship/Pick up Form

Directions for filling out the form:

1.  When filling out the form please print legibly.
2.  Be sure to include your middle initial when filling out your name.
3.  Be sure to include your EMPL ID.
4.  Make sure your address is a street address and not a PO Box. (for shipping purposes – shipping charges will be charged to your financial account.
5.  Please put a daytime and nighttime phone number where you can be reached.
6.  Be sure to put the email address you read most frequently on the form.
7.  Please indicate your method of receiving your textbooks (ship/pick up).
8.  Please circle yes or no if you want the recommended titles.
9.  Please circle the line if you want only new textbooks.
10. Be sure to include a copy of your class schedule when you turn in the form.
11. Please read the disclaimer and then sign and date.

Then return the form back to the VHCC Bookstore with a copy of your class schedule by faxing or dropping the form off at the Bookstore.
