Proctor Information Form


If you intend to take your tests at the VHCC learning lab, you do not need to fill out this form.  Otherwise you will need to supply the requested information.  Your instructor will then contact your proposed proctor to agree on procedures, and you will be informed by email when this occurs.  You will also be informed by email when the proctor replies.

Testing information will be sent to your proctor or to the testing center via email, and an email address is therefore necessary.

Some testing centers prefer that I send information to a central address so that all personnel at the center can access the information.  You should determine whether this is the case for your proctoring site, and if so you should provide that email address either in addition to or instead of the address of your specific proctor.
