FORMS

In order to be eligible for selection to attend the VHCC Summer Governor's School you must complete

the following forms:

        1.  Application for the session you plan to attend.

        2.  Have two (2) teachers complete the Teacher Recommendation Form for the session you plan to attend.

        3.  Have your Counselor complete the Assessment of Application Form for the session you plan to attend.

        4.  Complete the Student  Statement Form. Make sure parent or guardian signs this form

        5.  Complete the Field Trip Release Form. Make sure parent or guardian signs this form.

        6.  Complete the Medical Emergency Information Form. Make sure parent or guardian signs this form.
