Flu shots for employees, retirees and family members will again be provided by Mount Rogers Health District (MRHD). The cost of the flu shot this year is $25 and MRHD will file the claim with Anthem for employees who have insurance coverage. They have stated that they plan to accept the amount paid by Anthem under our wellness benefit and do not plan to balance bill. If you do not have insurance coverage, you may still participate and pay MRHD for the flu shot that day. Please preregister with Debbie Hale so that she can send you information about the flu shot and the MRHD form that each person will need to fill out to receive the immunization. Please bring the completed form and a copy (front and back) of your insurance card with you on the 15th. |
