Instructions for the Completion of the
COMPENSATORY/OVERTIME LEAVE EARNED FORM
These forms must be completed in either black or blue ink. No correction fluid
may be used. If it is necessary to make a correction on the time card, please neatly
draw a line through the erroneous entry and write the correct information above the error.
The employee's supervisor must initial all corrections.
Compensatory/Overtime Leave Earned forms are to be completed and signed only
after the hours are worked. The work hours may not be anticipated.
The forms should be submitted to the payroll office (Room 106, ADM Building) with the VHCC TIME RECORD CARD covering the period in which the
compensatory/overtime leave is earned.
It is the supervisor's responsibility to see that the forms are calculated
accurately and have been completed properly. Reminder: all
overtime hours must be submitted by supervisor to the VP of Administration in
advance for approval. The overtime request form is available on the
"F" drive.
Items to be completed are detailed below; you may click each area of the leave
earned form to go to references:

Number of hours (reported in tenths of a hour; see chart below) compensatory
leave earned OR
- Number of days compensatory leave earned.

Beginning time of compensatory leave earned.
- Ending time of compensatory leave earned.
- Date on which compensatory leave was earned.

Reason for working - REQUIRED. The reason for working
should generally fall under one of the three reasons listed. If it
does not, please detail
the reason in the "Other" section.
