Financial Aid Students

Students wishing to charge their textbooks and supplies to their financial aid account should complete the Financial Aid Textbook Ship/Pick up Form.

Directions for filling out the form:

• When filling out the form please print legibly.
• Be sure to include your middle initial when filling out your name.
• Be sure to include your EMPL ID.
• Make sure your address is a street address and not a PO Box. (for shipping purposes – shipping charges will be charged to your financial account.
• Please put a daytime and nighttime phone number where you can be reached.
• Be sure to put the email address you read most frequently on the form.
• Please indicate your method of receiving your textbooks (ship/pick up).
• Please circle yes or no if you want the recommended titles.
• Please circle the line if you want only new textbooks.
• Be sure to include a copy of your class schedule when you turn in the form.
• Please read the disclaimer and then sign and date.

Please return your form and a copy of your class schedule to the VHCC Bookstore.  The VHCC Bookstore is located at the Southwest Virginia Higher Education Center on the VHCC Campus.
